Achilles Tendon: Diagnostic & Treatment

The Achilles tendon is the thickest and strongest tendon in the human body. It connects the calf muscles (gastrocnemius and soleus) to the heel bone (calcaneus), acting as a powerful spring that allows us to walk, run, and jump. Despite its strength, the Achilles is highly vulnerable to injury because it lacks a robust blood supply and is subjected to forces up to twelve times your body weight during high-impact activities.

UNDERSTANDING ACHILLES TENDINOPATHY

The most common cause of pain in this area is Achilles Tendinopathy. This is not typically a simple “inflammation” (tendonitis), but rather a “failed healing response” (tendinosis) where the tendon fibers become disorganized and thickened. Because the tendon has a “watershed zone” with poor blood flow about 2–6cm above the heel, this specific area is the most common site for chronic pain.

Signs you may be suffering from Achilles Tendinopathy include:

  • Pain and stiffness at the back of the ankle, especially first thing in the morning.

  • A visible “lump” or thickening in the middle of the tendon.

  • Pain that “warms up” and disappears during exercise, only to return much worse afterward.

  • Tenderness when “pinching” the tendon between your fingers.

  • Difficulty performing a single-leg heel raise.

Treatment Approaches for Achilles Pain:

  • Load Management: Adjusting your training volume to keep the tendon within its “stress tolerance” window.

  • Progressive Strengthening (Eccentrics/Heel Raises): The gold-standard exercise approach to remodel the tendon fibers and increase their capacity.

  • Diagnostic Ultrasound Scan: To visualize the internal structure of the tendon, measure thickening, and rule out tears or retrocalcaneal bursitis.

  • Extracorporeal Shockwave Therapy (ESWT): A highly effective, non-invasive treatment that uses sound waves to stimulate blood flow and trigger a healing response in chronic tendons.

  • Biomechanical Assessment: Analyzing your foot strike and calf length to determine if “over-pronation” or tight muscles are overloading the tendon.

  • Heel Lifts or Orthotics: Temporary use of inserts to “offload” the tendon and reduce the mechanical pull on the heel.

  • Ultrasound-Guided Injections: Specialized procedures such as high-volume strips or peritendinous injections to reduce pain in cases where conservative rehab has plateaued.

  • Activity Modification: Guidance on cross-training (like cycling or swimming) to maintain fitness while the tendon heals.
