Shoulder Pain: Diagnostic & Treatment

Shoulder pain is rarely just “one thing.” Because the joint is so mobile, it relies on a complex balance of tendons and fluid-filled sacs. We use ultrasound to see exactly which structure is failing.

Shoulder pain can be debilitating, affecting your sleep and your ability to perform simple overhead tasks. At ProPlus Physio, we skip the guesswork. Our specialist clinic provides a comprehensive diagnosis using Musculoskeletal Ultrasound, followed by precision-guided treatments.

Effective treatment for these conditions often involves a combination of appropriate rehabilitation techniques and injections to reduce inflammation and pain, facilitating ongoing range of motion and strengthening exercises

UNDERSTANDING THE SHOULDER JOINT:

The shoulder is one of the most sophisticated and mobile joints in the human body, functioning as a “ball-and-socket” mechanism where the head of the humerus (upper arm bone) fits into the glenoid cavity of the scapula (shoulder blade). Because the socket is remarkably shallow—often compared to a golf ball sitting on a tee—the joint relies heavily on a complex network of soft tissues for stability. The primary stabilizers are the four muscles of the rotator cuff (supraspinatus, infraspinatus, teres minor, and subscapularis), which work in unison to hold the humerus centered in the socket during movement. This bony architecture is further supported by the labrum, a ring of specialized fibrocartilage that deepens the socket, and the subacromial bursa, a fluid-filled sac that reduces friction between the tendons and the acromion process. Together, these components allow for the immense range of motion required for daily tasks, but their intricate interdependence makes the shoulder particularly vulnerable to impingement, tears, and instability.

Common Culprits: What is Really Going On?

Shoulder pain is rarely a single-issue problem. To treat it effectively, we must first identify which structure is struggling. The most common conditions we diagnose include:

  • Rotator Cuff Tendinopathy & Tears: The rotator cuff is a group of four muscles that keep your arm bone centered in its socket. Over time, these tendons can become frayed (tendinopathy) or suffer acute tears, leading to weakness and pain when lifting the arm.

  • Frozen Shoulder (Adhesive Capsulitis): This is a frustrating condition where the joint capsule becomes thick, stiff, and inflamed. It typically moves through stages of intense pain followed by a “freezing” phase where motion is severely limited.

  • Subacromial Bursitis: Between your shoulder blade and the rotator cuff lies a small, fluid-filled sac called a bursa. If this becomes inflamed, it creates “impingement,” a painful pinching sensation whenever you reach overhead.

  • Calcific Tendonitis: This occurs when calcium deposits build up within the tendons. It can cause sudden, agonizing pain that feels significantly more intense than a standard strain.

Treatment Approaches for Shoulder Pain:

  • Rest or modified activities: Adjusting daily movements to allow inflamed tissues to settle.

  • Nonsteroidal anti-inflammatory medications (NSAIDs): Pharmacological support to manage pain and chemical inflammation.

  • Physiotherapy exercises: Targeted routines designed to improve flexibility, range of movement, and muscle strength.

  • Heat or ice application: Thermal therapies used under clinician guidance to manage symptoms.

  • Diagnostic ultrasound scan: High-resolution imaging used to precisely identify the underlying cause of pain.

  • Ultrasound-guided corticosteroid injections: Utilizing real-time imaging for enhanced accuracy to alleviate pain and improve joint function.

  • Shoulder manipulation under anesthesia: A procedure used to physically loosen restricted shoulder tissue in severe cases.

You don’t have to wait for a surgery list to get back to the activities you love. By combining expert physiotherapy with advanced diagnostic technology, we bridge the gap between “dealing with the pain” and “resolving the injury.”
